Need to cancel a policy?

We're sorry to hear you need to cancel your coverage.

The process for canceling a policy differs depending on how you pay your policy's premiums. Review your options below.

How is your policy paid?

You pay by payroll deduction

The first step is to contact your American Fidelity account manager. Your HR or payroll department can provide you with their contact information. There may be requirements for canceling outside of your annual enrollment opportunity or qualifying life events due to Internal Revenue Code rules.

You pay American Fidelity directly

If you are paying premiums directly to American Fidelity for your insurance coverage by credit card, EFT, or check, please complete a Cancellation Form to submit your request to cancel coverage.

Note: This form is not for cancelling policies that are deducted from your payroll.

Retiring or changing jobs?

Good news! You may not need to cancel your policy. You can keep many of our benefits when you retire or change employers, which can be helpful to avoid rising costs due to age or health evaluations.
