The fastest way to file an insurance claim is through your online account, or on our mobile app, AFmobile®.
- Online
- Log in to your online account.
- Click on the File a Claim button.
- Click the policy you want.
- Upload the requested documentation and click the Upload File button.
- Click the Next button and enter the remaining information on the following pages.
- Check the acknowledgment box and click on the Submit button.
- AFmobile
- Tap the File a Claim button on the AFmobile dashboard.
- Select the policy you are filing a claim and tap the Choose Files button.
- Either snap a photo of your documentation and tap the Attach Documents button or upload it from your mobile device and hit Next.
- Follow the prompts and fill out the required information.
- Check the acknowledgment box and click the Submit button.
- Fax or Mail
- Download and complete the relevant claim form from our claim form page. Please note, paper claim filing is not the fastest option. File a claim online or through AFmobile to get your money faster.

- Online
- Log in to your online account.
- Click the File a Claim button.
- Select Reimbursement Account(s).
- Complete the claim form.
- Click the Submit button.
- AFmobile
- Tap the Submit Reimbursement button on the AFmobile dashboard.
- Complete the Claim Form.
- Click the Submit button.
- Fax or Mail
- Download and complete the relevant claim form from our claim form page. Please note, paper claim filing is not the fastest option. File a claim online or through AFmobile to get your money faster.

The fastest way to check the status of a claim is through your online account, or on our mobile app, AFmobile.
For Insurance Claims:
- Online
- Log in to your online account.
- Click the Claims tab.
- AFmobile
- From the AFmobile dashboard, scroll down to Insurance Claims.
- Choose either In Process or Processed.
- When you file a claim, it will be located under In Process until it has been processed.
- Once your claim is processed, it will be located under Processed.
For Reimbursement Claims:
- Online
- Log in to your online account.
- Click the Reimbursements tab.
- Select View Claim Activity.
- Green claims are approved.
- Blue claims require action, such as uploading required documentation or more information.
- Red claims are denied. Click on the transaction for details.
- AFmobile
- Log in to AFmobile.
- Select the reimbursement account you wish to review.
- Select a claim from the transactions list.
- You'll also find the claim status, amount, and other details here.
- Online

For Insurance Claims:
- Online
- Log in to your online account.
- Click your name in the top right of the screen to access My Profile.
- Scroll down and click the Direct Deposit tab.
- Enter or update your bank account information.
- Click the Save Direct Deposit button.
- AFmobile
- From the AFmobile dashboard, go to your Profile.
- Under Direct Deposit, select Insurance Direct Deposit.
- Follow the prompts to set up your account and click on the Save Direct Deposit button.
For Reimbursement Claims:
- Online
- Log in to your online account.
- Click the Manage My Reimbursement Account button.
- Locate the Sign Up for Direct Deposit section and click Add. Or, hover over your name and click Profile.
- Click Direct Deposit and enter your bank account information.
- Click Save.
- AFmobile
- From the AFmobile dashboard, go to your Profile.
- Under Direct Deposit, select Reimbursement Direct Deposit.
- Follow the prompts to set up your account and click on the Save Direct Deposit button.
- Online

- Log in to your online account or AFmobile.
- Select the Additional Documentation button.
- Select either Reimbursement Account(s) or Insurance Claim.
- For Reimbursement Accounts, select the pending transaction and click Add Receipt to upload documentation.
- For Insurance Claims, select the insurance policy from the dropdown, then upload your documentation.

Claims are generally processed within seven business days after all required documentation is received.
Note: If you have an American Fidelity cancer, accident, critical illness, or disability policy, most annual wellness and screenings exams, diagnostic testing, health screenings, or physician expense benefit claims may qualify for quick processing. When you submit one of these claims online or through AFmobile, you can receive payment in as little as one day when enrolled in direct deposit. No extra documentation is required.
