TIP: To file your claim, you need to provide an Attending Physician Statement. This form must be completed by the doctor in charge of your care. Your employer must also complete the Employer's Report of Claim form, which they can do within their online portal account.

  • Disability Income Insurance is designed to pay you a portion of your salary if you have to miss work due to a covered injury or illness.

  • A disability is an inability to perform the duties of your regular occupation for the period stated in your policy. After this period, disability is an inability to perform duties of any occupation based upon experience, training, or education. For consideration of disability, you must discontinue working, remain under regular and appropriate care of a physician, and provide proof of loss through the claims process.

  • Benefits are paid directly to you, so you may use the money toward medical costs or daily living expenses as needed.

  • The period of time specified within the policy an insured must be disabled and in which no benefits are payable. Log in to view your specific policy details 

  • The disability payment refers to the Disability Benefit purchased minus a Deductible Sources of Income within your policy. Sources of income which may impact your disability payment include but are not limited to the following:

    • Sick leave/salary wage continuance
    • Social Security benefits
    • Other group disability insurance benefits
    • Government or other retirement income
    • State disability (if applicable)
    • Unemployment compensation
    • Workers compensation (if applicable)

  • A condition for which you have received treatment, incurred expense, taken medication, received services or a diagnosis or advice from a physician during the period described in the policy immediately before the effective date of coverage.

  • If you received disability insurance benefits, your employer will provide the tax forms and information needed for tax filing. In some circumstances, you may receive a W-2 form directly from American Fidelity to your mailing address.

  • Yes, the policy may include exclusions for disabilities such as intentionally self-inflicted injury, acts of war, disabilities contracted while in the services of the armed forces, periods of incarceration, and work-related injuries/sickness. Log in to view your policy details

      1. Log in to your online account or open your AFmobile app.
      2. Click the File a Claim button.
      3. Select the type of claim you would like to file.
      4. Follow the prompts to provide claim details and required documentation.
      5. Check the acknowledgment box and click the Submit button.

  • When filing a disability insurance claim, you will need to provide the following documentation:

    • Statement of Insured, completed through your online account during the claim filing process
    • Attending Physician Statement: The physician in charge of your care must complete this form. You can upload the form via your online account, or you can mail or fax it.
    • Employer's Report of Claim: After your employer completes this form, you can upload the form via your online account, or you can mail or fax it. If applicable, your employer can upload the form themselves via their own online account.


    If you're an employer, follow the instructions below to complete the Employer's Report of Claim form online:

      1. Log in to your portal account.
      2. Select the Claims page.
      3. Scroll or search for the employee you wish to complete the form for
      4. Click Need under the Employer Form column to start the process.


    Note: The Employer's Report of Claim statement may not be required when filing for a routine pregnancy.


    We may also need:

  • Claims are generally processed within seven business days after all required documentation is received.

  • The fastest way to check the status of a claim is through your online account, or on our mobile app, AFmobile®.

    • Online
      1. Log in to your online account
      2. Click on the Claims tab
    • AFmobile
      1. From the AFmobile dashboard, scroll down to Insurance Claims
      2. Once a claim has been processed, it can be found in this section under the insurance policy name.
  • You do not have to submit the forms all together. Your claim will be reviewed for processing once all required forms are received.

  • Your doctor and/or employer can submit their portion of the claim form themselves.

  • If your disability has extended beyond what was originally approved, you and your treating physician must complete a continuing claim form.

  • The Employer’s Report of Claim should be completed by your payroll representative. 


    If you're an employer, follow the instructions below to complete the form online:

    • If your employee has started the claim and needs your portion of the form: (This means your employee has started the claim process in their online account or by mail.)
      1. Log in to your portal account.
      2. Select the Claims page.
      3. Search for the employee under Incomplete Disability Claims.
      4. Select the green Need button under the Employer Form column for the employee.
      5. Follow the prompts to complete the digital employer form.


    If your employee would like you to submit their statement of insured or physician form on their behalf, follow the instructions below for uploading documents. If your employees set up text or email alerts, they'll receive a confirmation that their claim has been received. If additional information is needed, we’ll contact the policyholder.

     

    • If you are starting a new disability claim on your employees' behalf:
      1. Log in to your portal account.
      2. Select the Claims page.
      3. Click the Provide Claim Information button.
      4. Search for the employee for whom you wish to upload documentation.
      5. Select the employee's name and click Submit. Then, upload your documentation.


    You can upload forms including the statement of insured, employer report of claim, and a physician statement. If your employees set up text or email alerts, they'll receive a confirmation that their claim has been received.

  • Unfortunately, American Fidelity does not pay for claim forms to be completed. It is the insured’s responsibility to get the claim forms completed.

    • Log in to your online account
    • Select the File a Claim button
    • Select Physician Expense
    • Complete the claim form information, then select Next
    • Select the ready to file my claim button
    • Review your information, then select Next
    • Complete the authorization, then select Submit

  • A document that details information about specific plan benefits. You can view your Schedule of Benefits in your policy document through your online account.

  • The period of time for which benefits may be payable for a covered injury or sickness. Benefits may be payable up to the maximum Disability Benefit Period described in the policy. Log in to view your policy details 

  • Yes, the policy may include limitations for certain health conditions. Log in to view your policy details 

  • Benefits are taxable if any portion of your disability premiums are paid by your employer or if your disability premiums are withheld before tax (like with a Section 125 Plan). If your disability premiums are withheld after tax and the employer does not pay any portion of your disability premiums, the benefit is not taxable. Please refer to your employer to determine if your premiums are withheld before or after tax.

  • The plan pays a percentage of your salary. The percentage is based on the plan selected by your employer. In general, most disability plans cover between 60% and 70% of your covered salary amount, not to exceed the plan maximum benefit amount.

    Note: Your coverage does not automatically change with your salary. If your salary has increased since your last enrollment, take the opportunity to increase your coverage amount as well.
