How do I file a Physician Expense claim?

1. Log in to your online account
2. Select File A Claim, then select Insurance Claim
3. Select Physician Expense
4. Complete the claim information, then select Next
5. Select the ready to file my claim button
6. Review your information, then select Next
7. Fill out and complete the authorization section, then select Submit
