Where can I find my employees' election forms?

To view election forms, follow these steps:

1. Visit the Files page. 
2. Select the Location from the drop list. Below the main folders will be Election Forms. 
3. Select the Plan Year and Search by Employee Name. 

Forms can be viewed, printed or saved for your convenience.
