How do I file a non-routine pregnancy claim?

If you experience complications and must be out of work longer than 6 weeks (or 8 weeks for a C-Section), this is considered a non-routine pregnancy claim. In this situation, please have your treating physician complete the Attending Physician Statement and outline the complications that are preventing the release to return to work. You should also include a copy of the office notes which document the diagnosis and treatment for the complications.

Tip: Your claim should be submitted after you become disabled and have stopped working.
