Uncashed Checks / Unclaimed Property

Most often, unclaimed property consists of funds or checks that are left uncashed or uncollected for several years.

This can happen when checks are sent to a previous address or simply misplaced. Whatever the reason, we’re here to help you claim what’s yours.

We recently sent you a letter to inform you about the payment and confirm your interest in the property or funds. American Fidelity processed the payment on the date mentioned in the letter and our records indicate this payment (check) was never cashed.

Note: Checks are valid for 90 days. If a check is not cashed within 90 days, it will require a new request to reissue.

If you changed your name, please send evidence documenting the name change along with your completed claim form by the date specified in the letter. Documentation can include:

  • A marriage certificate
  • A divorce decree
  • A new driver's license
  • Adoption records

The named beneficiary will need to:

  1. Fill out the claim form containing the beneficiary's information, including a current mailing address.
  2. Include a copy of the deceased’s death certificate and a copy of the beneficiary's driver’s license or state-issued ID.
  3. Email UnclaimedProperty@americanfidelity.com, fax 405-523-5847, or mail your completed form to:

If your letter specifies it is a final notice, we must receive the completed claim form by the deadline listed on the letter. We are required by law to send the money to the state government after the deadline on the letter. At that point, you will need to claim your funds directly from your state’s unclaimed property department.

The first letter you receive is considered a courtesy letter. We are not required to send the money to the state following the deadline listed in that letter. As a result, we may still be able to reissue the check past the due date. A courtesy letter will have no mention of a "final notice."

Please email us at UnclaimedProperty@americanfidelity.com to ask if the funds are still available for reissue.

Please allow 60 days for a new check to be mailed to you.

Since some funds may include pre-tax contributions in our system, the check must be made out to the employer instead of the employee. If needed, we can also reissue any checks that went uncashed. 

Please advise your employer:

  1. Fill out the claim form 
  2. Email UnclaimedProperty@americanfidelity.com, fax 405-523-5847, or mail the completed form to:

Since some funds may include pre-tax contributions in our system, we are unable to reimburse or reissue a new check to the respective employee. In these instances, employers cash the employer check(s) then decide how to pay the amount to the associated employee.
