Documentation Needed to File a Claim
If you’re a beneficiary who has recently lost a loved one, we want to make the claims process as easy as possible. Life insurance claims cannot be filed online but must be mailed, faxed or emailed to AFA-life-claims@americanfidelity.com. You’ll need to provide these documents:
- Claim Form: Download, complete and sign this form.
- Certified Original Death Certificate: Must show the deceased's cause and manner of death and mailed to American Fidelity.
- IRS Form W-9: Must be completed for each payee.
- Direct Deposit Form: To have your claim benefit direct deposited, please complete this form.
Customer Service Policy Changes:
- Policy Change Request Form: Download, complete and sign this form.
- IRS Form W-9: This form is required for ownership changes, name changes, policy surrenders/cancellations, annuity partial withdrawal or surrenders, and loans.
