How do I file an insurance claim?

To file an insurance claim online or using AFmobile:

1. Log in to your account online or through your mobile device.
2. Click the File a Claim button from your dashboard.
3. Select the policy you want to file a claim for.
4. Use the Choose Files to Upload button to attach the requested documents. Click Next.
5. Fill in the required claim information where prompted.
6. Verify that your personal information is accurate.
7. Check the acknowledgment box and click Submit to finalize your claim.

To file an insurance claim by fax or mail:

1. Download and complete the relevant claim form from our Claim Form page.
2. Submit the completed form and required documentation via fax or mail.
