How do I file a Wellness or Health Screening Benefit?

  1. Log in to your online account
  2. Select File A Claim, then select Insurance Claim
  3. Select Wellness/Health Screenings
  4. Complete the claim information, then select Add Test, then Next
  5. Review your information, then select Next
  6. Fill out and complete the authorization section, then select Submit
