How do I file a disability pregnancy claim?
- Log in to your online account
- Select File A Claim, then select Insurance Claim
- Select Pregnancy
- Complete the Delivery Info, Medical Provider, Hospital Info and Income & Payments sections
- Review the information you provided, then select File the Claim, then complete the Fraud Statement
