Reporting Requirements
You can fulfill your reporting requirements to the state via My FAMLI+ Employer website or by using paper forms.
Here is a brief overview of the requirements.
Quarterly / Annual Reporting
The Colorado Family and Medical Leave Insurance (FAMLI) Division requires that private plan administrators, like American Fidelity, file a joint quarterly report on all equivalent private plan data. This report includes information about all our Paid Family and Medical Insurance (PFMLI) plans in Colorado, including yours. No action is necessary from you. It is our responsibility to gather this information and file it quarterly for the first three years and yearly thereafter.
Annual Private Plan Attestation (APPA)
Beginning in November 2024 and every November thereafter, employers with approved private plans must annually submit an attestation to the FAMLI Division that their contact information is accurate and their approved private plan continues to satisfy the requirements of the FAMLI Act and its implementing regulations. Failure to submit the annual attestation via the My FAMLI+ Employer website may result in the Division's withdrawal of the private plan approval.
