You can request leave through your online account at americanfidelity.com/login or our mobile app, AFmobile®.

If your employer uses the American Fidelity PFMLI plan, you should be able to request leave in your online account. Please contact your account manager or call us at 800-662-1113 for support if you do not see this option.

If you know you need to take paid leave, inform your employer at least 30 calendar days beforehand, or as soon as possible if 30 days’ advance notice is not possible. You can apply for PFMLI benefits up to 30 days before the date leave is expected to begin. You will need to update American Fidelity once you begin your leave because benefits will not be paid until after your leave starts.
Advance notice may not be possible under certain circumstances. If the leave is unexpected, you should still provide notice to your employer as soon as possible under the circumstances. You can apply for PFMLI benefits up to 30 days after your leave has started.

You may take up to 12 weeks per benefit year for any combination of approved leave:
- Medical Leave: Caring for your own serious health condition
- Family Leave: Bonding with a child within the first year after birth or placement through adoption or foster care or caring for a family member with a serious health condition
- Safe Leave: Seeking help as a covered individual or for a minor child/dependent, related to domestic violence, stalking, sexual assault or abuse
- Military Family Members (Qualifying Exigency) Leave: Leave to dealing with a need arising out of a covered individual’s family member’s active duty service or notice of an impending call or order to active duty in the armed forces
An additional four weeks of paid leave may be available for a serious health condition relating to pregnancy or childbirth complications.

To be eligible for American Fidelity’s Paid Family and Medical Leave Insurance (PFMLI):
- You must work for a qualified employer and be considered a qualified employee under Colorado’s FAMLI program.
- You must have earned at least $2,500 in wages subject to PFMLI premiums during the covered individual’s base or alternative base periods.
- The $2,500 earnings threshold can be met by wages from any combination of qualified employers.
- It is not necessary to have earned $2,500 from your current employer to fulfill the earnings requirement.

Benefits will depend on your earnings compared to Colorado's Average Weekly Wage. Workers will receive 90% of their average weekly wage for the portion of wages that is equal to or less than 50% of the Colorado Average Weekly Wage, and 50% of their average weekly wage for the portion that exceeds the Colorado Average Weekly Wage, subject to the maximum weekly benefit of $1,100 for 2024.
Here is an example based on annual earnings.

Contributions are .9% of your wages. Your employer is responsible for at least 50% of your monthly premium. Any remaining portion not covered by your employer will be taken from your paycheck.

Yes. Intermittent or reduced schedule leave is available when supported by required documentation. You’ll work with your employer, medical provider, and/or other individual certifying your need for leave to determine what type of leave is best for your situation.

It’s possible. We recommend speaking with your account manager to discuss your retirement plan’s rules and how paid leave may affect your retirement goals.

Paid leave will only pay 12-16 weeks each benefit year. Disability Income Insurance gives you protection beyond, and sometimes in addition to, your paid leave eligibility. It’s important to talk to your account manager to learn how paid leave and disability work together.

To register, go to the login screen, then click Register. You’ll need your Customer Number or Social Security number. Fill out the requested information and create your username and password.
Note: Your username and password are the same for both AFmobile and online.

To reset your password, go to the login screen and click Forgot Password. Input your username, choose your preferred communication method to receive a one-time code and follow the prompts. You'll need to reset your password before logging in to your account.

Log in to your online account. Click the profile icon, then click Password. Click Save Password when you’re done.

Family member's serious health condition

New parents filing a Parental (Bonding) Leave claim do not need their claim verified by a health care provider. Instead, they will need to upload proof of birth or placement, which must include the child’s birth date or placement date. Examples may include, but are not limited to:- A birth certificate or verification of birth worksheet
- Birth or recovery care documentation from a health care provider or medical facility (make sure your proof of birth documentation includes your child's date of birth)
- Adoption or foster care placement paperwork
Expecting parents can start their Parental (Bonding) Leave application up to 30 days in advance of their expected leave start date. However, the proof of birth documentation is required to fully submit the claim. Your claim will be saved as a draft until that documentation is submitted.
New parents are eligible to take PFMLI leave to bond with a new child anytime within the first 12 months after a birth, adoption or foster care placement. So parents who welcomed a new child in 2023 may have some time of eligibility in 2024 (when benefits became available), even if they used some employer-provided leave or unpaid FMLA leave in 2023 when the child first arrived. - To determine whether an individual is the survivor of domestic violence, the survivor of stalking, or the survivor of sexual assault or abuse, for purposes of determining eligibility for safe leave, an individual doesn't need to prove that a court has determined that the covered individual or a qualifying family member was the victim of domestic violence, stalking, sexual assault, or sexual abuse.
- Benefits may be awarded and will require the good faith, legal attestation that the covered individual or a qualifying family member is a survivor of domestic violence, stalking or sexual assault or abuse

- Proof of the service member’s active duty service
- Service member’s active duty military orders
- For all reasons an individual applies for PFMLI leave, American Fidelity may request additional reasonable information or documentation necessary to process the claim
