How do I file a claim for Life Coverage?

We must receive:

  1. a completed claim form,
  2. a certified copy of the insured's death certificate.
  3. Letters of Administration or Letters Testamentary, if claim is being filed by an Executor or Administrator.


PLEASE NOTE:  If you are a current Life Division or former Mid Continent Life Policyholder, you can obtain your life claim forms here.  Select your state and choose the Industry labeled "Other".
